Register at Stokkeland GP practice.

Do you live in postcode area 1431 and are not yet registered at a GP practice in Aalsmeer? Then you can register at Huisartsenpraktijk Stokkeland. You can easily register via this application form.

If you live outside postcode area 1431, we would like to ask you to contact the practice first. The assistant will assess whether you fall within the practice's catchment area.

Practical information about your registration

  • At your first visit, we ask you to bring valid identification (passport or ID card) and your health insurance details.
  • By registering with us, you consent to be deregistered from your previous GP. We also request that you inform your current GP yourself that you are switching.
  • Please note that filling in and sending the registration form does not automatically mean you are enrolled immediately. After we receive your form, we will contact you as soon as possible to let you know whether registration is possible.
