Help us improve care - fill in the questionnaire

It is important to us that you receive good and pleasant care at our GP practice. That is why our practice is assessed once every three years by the NPA. This is an independent organisation of the Dutch College of General Practitioners (NHG) that checks whether GP practices meet quality standards.

An important part of this assessment is your opinion. With the patient questionnaire, you let us know what goes well and what you think could be improved. This way, we can continue to improve our care and better tailor it to what you find important.

Completing the questionnaire will take about 5 to 10 minutes. Your answers will be processed completely anonymously.
