Chronic care is the care and support people need when living with a long-term or permanent condition, such as diabetes, asthma, cardiovascular disease, COPD, rheumatism or chronic pain. This care focuses on managing the condition in the long term, maintaining the best possible quality of life and preventing deterioration.
