Practice support physician for the elderly (POH-O)

Mr Groot increasingly loses track of things and sometimes wanders around the neighbourhood. Mrs De Vries hardly dares to go shopping alone since she fell with her walker. These are the people who come to you for consultations. Elderly people with their own stories, worries and wishes. As a POH-O, you take the time to listen, ask questions and look together at what is needed to keep life safe, pleasant and as independent as possible. This is exactly what makes working as a POH-O at Stokkeland so valuable.

This is what you will do

As POH-O You ensure that vulnerable elderly people are seen, heard and properly supported. You assess their situation, proactively organise the right care and always focus on safety and quality of life. You are close to the elderly and their network and guide, motivate and support them every step of the way. You also connect GP care with informal carers, elder care and the social domain. You steer towards integral care: one care provider within one organisation who coordinates the care and attunes it to the wishes and needs of the elderly person.

Your daily tasks

  • You will map all over-75s in the practice and identify which elderly people need extra support. Together with the elderly person (and family), you make a plan to stay independent and vital for as long as possible.
  • You give advice on lifestyle, exercise, nutrition and medication use - in a way that suits the older person's lifestyle.
  • You actively collaborate with other healthcare partners to address healthcare issues quickly and intelligently.
  • You make home visits to assess the home situation and identify timely risks or concerns.
  • You support and advise informal carers and keep an eye out for possible overload within the elderly person's network.

About general practice Stokkeland

GP practice Stokkeland is located in the centre of Aalsmeer, in a health centre where several disciplines work together. Think pharmacy, physiotherapy and a lab, all under one roof. This makes the lines of communication short and makes it easy to optimise patient care.

In the practice, you work with a committed and friendly team of three GPs, four practice support workers (somatic, mental health and youth) and three assistants. Consultations are easy, everyone thinks along with you and there is always room for sociability - and good coffee.

  • You have completed (or will soon complete) the POH-O course.
  • You have an eye for people. You quickly notice when someone feels differently and take it for granted to keep asking.
  • You communicate easily - with the elderly, family and care partners - and do so in a warm and clear manner.
  • You keep an overview, work in a structured way and enjoy bringing clarity to sometimes complex situations.
  • You are collaborative by nature. You seek out colleagues, think with them and enjoy organising the best possible care together.

  • Strong salary: Between €3,795 and €4,977 for a 38-hour working week (scale 7, CAO GP Care).
