What personal data do we process?
We process (depending on your situation) among other things:
- Identification and contact details: name, address, place of residence, telephone number, e-mail address, date of birth;
- Care and health data: complaints, diagnoses, results, medication, allergies, treatment data;
- Administrative data: insurance details, claims data and (where necessary) financial settlement data;
- Communication data: appointments and moments of contact (e.g. by phone or via digital patient environments - where applicable).
What do we use your data for?
We only use your personal data for clear purposes, such as:
- Care provision: providing GP care, maintaining medical records, referring and consulting with other healthcare providers;
- Practice management and administration: planning/arrangements, quality policy, administration and (where necessary) declaration to health insurer;
- Legal obligations: for example, notifications required by law (such as for certain infectious diseases), or other obligations imposed on healthcare providers;
- Quality improvement, teaching or research (only if allowed): only if allowed by law and/or with appropriate safeguards (e.g. anonymise/pseudonymise data).
We do not use your data for commercial marketing purposes.
On what bases (legal grounds) do we do this?
We process your personal data only when permitted under the AVG. We do this on the basis of:
- execution of the treatment agreement (provision of care);
- a legal obligation (e.g. obligations under the WGBO and other healthcare legislation);
- Your consent, when consent is required (e.g. for certain types of data sharing);
- a legitimate interest, such as keeping our practice safe and well organised (where we always consider your privacy).
The AVG requires us to explain these legal basis(s) to you.
Do you need to provide your details?
To provide good care and maintain medical records, certain data are necessary. If you do not want to provide necessary data, we may not be able to treat you (fully) or care may not be as well targeted. This is part of the information we must provide according to the AVG.
With whom do we share your data?
We only share your personal data:
- if this is necessary for your treatment (e.g. pharmacy, acting GP, medical specialist, GP out-of-hours surgery);
- If we have to do so legally;
- or if you have given your consent (where required).
We do not share more than is necessary for the purpose (‘data minimisation’).
Where applicable, data exchange may take place via secure systems (e.g. secure care email or the National Switch Point (LSP)), and only according to the applicable rules and your choices/consent.
Processors (suppliers)
For systems and ICT (such as the HIS, hosting, secure mail and other suppliers), we use service providers who process personal data on our behalf. We make agreements with these parties on confidentiality, security and privacy (processor agreements).
Referrals and care mediation
If you are referred to another healthcare provider, we will only send along data needed to follow up on the referral.
Upon referral, you are given the digital option to indicate whether you want to use mediation by your health insurer in case of excessive waiting times. Only if you give your explicit consent will data be shared with the health insurer. When giving permission, you will see which data will be shared.
This means: your healthcare insurer can only help you find a healthcare provider with a shorter waiting time if you want it yourself. No data will be shared with your health insurer for care mediation without your consent.
Juvoly Quickconsult (speech to summary)
In our practice, we use Juvoly Quickconsult. This tool converts (parts of) spoken text during the consultation into a draft summary for the GP Information System (HIS).
- The spoken text/audio is not saved.
- Only the summary checked by the GP will be included in your medical record.
Do you not want Juvoly Quickconsult to be used during your consultation? If so, please indicate this to your GP.
Cookies and use of the website
On the website of Huisartsenpraktijk Stokkeland, we use cookies. A cookie is a small text file that is stored in the browser of your computer, tablet or smartphone the first time you visit the website.
We use cookies that:
- needed for the website to function properly;
- help us understand the use of the website so that we can improve it.
These cookies do not collect medical data and, where applicable, are processed anonymously or pseudonymised. Data collected via cookies cannot be directly traced back to you as a person.
On your first visit to our website, we inform you about the use of cookies and - if necessary - ask for your consent. You can always change your cookie settings via your internet browser. You can also delete previously stored cookies via your browser settings.

Security
We take appropriate technical and organisational measures to secure your data against loss, unauthorised access or misuse. These include secure systems, access rights and logging (keeping track of who has accessed the file).
Do you suspect that your data is not properly secured or has been misused? If so, please contact the practice.
How long do we keep your data?
We do not keep your personal data longer than necessary. For medical records, the main rule under the WGBO is a retention period of 20 years from the time the record was last changed (unless longer retention is necessary, e.g. for good care or if data are important to others).
Your privacy rights
You have the following rights (subject to conditions):
- access to your personal data and a copy;
- rectification (adjustment) if data are incorrect;
- erasure/deletion or (partial) destruction of medical data, if permitted and not in violation of legal retention obligations;
- limitation of processing;
- object to certain processing operations;
- data portability (data portability) in situations where this applies;
- withdraw consent (if the processing is based on consent).

Complaint
Not satisfied with how we handle your personal data? If so, please contact the practice. You also have the right to lodge a complaint with the Personal Data Authority at www.autoriteitpersoonsgegevens.nl.
Changes
We may amend this privacy notice if necessary (e.g. in the event of changes in practices or legislation). The current version can be found on our website.
