Your privacy

Huisartsenpraktijk Stokkeland handles your personal data with care. It is important to us that your privacy is well protected. That is why we follow the rules of the General Data Protection Regulation (AVG) and the Medical Treatment Agreement Act (WGBO).

These laws set out how we handle your data. Your data is only used for the purpose for which you have given it. We also ensure that your data is properly secured and treated confidentially. After all, it concerns your personal and medical information.

What data do we use

In our GP practice, we process personal data such as your name, address, date of birth and medical records. This is necessary to provide you with good care. We also use data for administration, e.g. sending declarations to your health insurer.

Sometimes we are obliged to use or share data because of the law. This may be the case, for example, when reporting an infectious disease or in case of serious danger to your health or that of others.

What we use your data for

Your data will only be used for the provision of care and the smooth running of the practice. This means that we use your data to record consultations, carry out treatments and maintain contact with you. In some cases, data may also be used to improve care, education or scientific research, but only if permitted by law.

Your data will not be used for other purposes.

Confidentiality and security

All staff at Huisartsenpraktijk Stokkeland have a duty of confidentiality. They may only view or use your data as necessary for their work.

We keep your data secure. We do this with secure computer systems and clear agreements with suppliers who help us with ICT. We also keep records of who views your file and when.

We do not keep your medical data longer than necessary. By law, we usually keep medical data for 15 years after your last treatment. Sometimes it may be necessary to keep data longer, for example for your health or that of your children. This is decided by the GP.

Your rights

You have the right to know what personal data we hold on you. You may inspect your file and request a copy. If data is incorrect, you can ask to have it amended. In some cases, you can also ask to have data deleted or destroyed, as long as this is permitted by law.

In addition, you may have your own medical statement added to your record. You can also object to the use of your data in certain situations.

Sharing data with others

We share your personal data with others only when necessary for your care. We usually do this only with your permission. Sometimes we may or must share data without consent, for example if the law requires it or in case of serious danger to your health.

Your data may be shared with other healthcare providers, such as the pharmacy, an acting GP or a medical specialist. This is always done carefully and only with the information needed.

Juvoly Quickconsult

In our practice, we use Juvoly Quickconsult. This is a digital tool that helps create a summary for the GP Information System (HIS) during the consultation. The tool converts spoken text from the conversation into a draft summary.

The conversation can be (partially) processed for this purpose. The spoken texts themselves are not stored. Only the summary, after the GP has checked it, is recorded in your medical record in the HIS.

Juvoly Quickconsult is only used to record the consultation properly. It helps the GP to type less and pay more attention to you during the consultation. If you do not want this tool to be used during your consultation, or have any questions about it, please feel free to tell your GP.

Transfer of your file

If you switch to another GP, it is important that your new GP knows your medical history. This is why your old GP transfers your file to your new GP. This is usually done digitally and within one month of your request.

You cannot bring the original file yourself. However, you are always entitled to inspect and have a copy of your file.
